• Refer to pain management if:
  • Patient is known to pain management service and open to supported self-management
  • Previously had physio input with no benefit and imaging did not show any surgical target

• Refer to physiotherapy if:
• The patient has not had physiotherapy before
• The patient requests re-referral
• The patient had benefit from physiotherapy in the past
